How to Submit Your Application
Follow these steps to submit your pollution or environmental liability insurance application. Questions? Email us at quotes@pollutionliabilityinsurance.com.
Download the Correct Application
Visit pollutionliabilityinsurance.com/applications and download the application for your coverage type:
- Contractors Pollution Liability (CPL) — for contractors, construction, HVAC, demolition, and trade work
- Site-Specific Environmental (EIL / PLL) — for property owners, developers, and lenders
- Storage Tank Liability — for underground and aboveground storage tank owners and operators
- Environmental Consultants E&O — for environmental consulting firms and professionals
- Combined GL / CPL / E&O — for contractors needing bundled general liability and pollution coverage
- Excess Environmental — for additional limits over an existing primary environmental policy

Complete the Application
Fill out every section as completely as possible. Incomplete applications slow down the quoting process. A few tips:
- Use the legal name of the business or entity applying for coverage
- Include revenue, payroll, or square footage as requested — underwriters use this to size the risk
- Describe operations clearly — vague answers may result in a request for more information before a quote can be issued
- If there is a prior pollution claim or known contamination, disclose it — carriers will ask and surprises hurt quotes
- Sign and date the application where required
Working With an Insurance Agent? Include Their Information.
If you have an insurance agent or broker helping you with this placement, include their contact details in your email. This allows us to coordinate with them on your behalf and send the quote directly to them to review with you.
In your submission email, include your agent's:
- Full name (required)
- Email address (required)
- Phone number (optional)
- Agency name (optional)
Example: "My agent is Jane Smith at ABC Insurance — jane@abcins.com, (555) 123-4567. Please copy her on the quote."
No agent? That's fine — we can work with you directly. Just leave this step out.
Email Your Completed Application
Send the signed, completed application as an email attachment to:

Example: Application — Smith Environmental Services — CPL
What to attach: Your completed and signed application (PDF preferred). Include any supplemental forms, loss runs, or schedules of locations if requested by the form.
In the email body, include:
- Desired effective date (or requested start date)
- Coverage limits and deductible preferences, if you have them
- Any special requirements (e.g., additional insured, certificate holder)
- Your agent's info if applicable (see Step 3)
Expect a Response Within One Business Day
Once we receive a complete application we will:
- Send you (and your agent, if provided) a confirmation that your application was received
- Submit to our A+ rated carrier markets for pricing
- Respond with one or more competitive quote options, usually within one business day
- Follow up if we need any additional information to complete the submission
Complex risks or specialty programs may take 2–3 business days. We'll let you know if your submission requires additional review time.
